Investment Change/Contribution Change Forms

Below are the necessary forms that will allow you to:

	Increase or decrease the amount you are contributing
	Change the direction of your future contributions
	Transfer balances among the different investment options

If you want to Increase or Decrease the amount you are contributing to your 401(k) plan you will need to complete a Participant Contribution Election Change Form:

	Click here if your plan allows for Traditional 401(k) contributions only. Click here if your plan allows for Roth 401(k) and Traditional 401(k) contributions.
	Upon completion, forward the form to your Human Resources/Payroll department.

If you want to change the direction of your Future Contributions, you can:

	Complete a PARTICIPANT INVESTMENT ELECTION CHANGE form; OR
	Login to Internet SAM and select INVESTMENT ELECTIONS

	Complete a PARTICIPANT INVESTMENT ELECTION CHANGE form; OR
	Logon to Internet SAM and select TRANSFER FUNDS, REBALANCE PORTFOLIO or CONFORM TO TARGET

If you decide to complete a PARTICIPANT INVESTMENT ELECTION CHANGE form, upon completion, mail OR fax the form to:

will be made as soon as administratively possible after the form has been
